Things to know before you go.

Cancelation and No-show Policy

Dear valued clients, when booking any appointments with us please be aware and respect our fair cancelation policy.

Cancel for free up to 24 hours ahead, otherwise you will be charged 10% of the service price for late cancellation, 30% for not showing up the first time and 100% for the second time.

If you arrive more than 15 minutes late we may not be able to accommodate you. Fees will apply.

If you cancel within an hour of your appointment the no show policy will apply.

Any fees must be paid prior to rebooking.

We understand that some things may be out of your control so we may wave or reduce fees based on circumstances.

If no shows and late cancelations become a habit, you may not be able to rebook with us.

If you have a cellphone number on file, you will receive a text reminder 24 hours prior to your appointment. This reminder includes a link to fresha with our policy. Do not reply to these texts as they are automated; we will not see them.

If you have an email on file you will receive a confirmation upon booking and a reminder 24 hours and 72 hours before your appointment. These emails will include our policy and a link to change your appointment if need. You may reply to these emails.

If you aren’t receiving these notifications and need to cancel or reschedule, please contact the shop directly.

Client Forms

If you are a first time client or haven’t filled out your yearly health history form, please arrive 5-10 min early to your appointment to fill one out. You can also click on any of the forms below to download a PDF. You can print and fill this out at home and bring it to your next appointment.

Any client who is under the age of 18 must have a parent or guardian sign a consent form in person. This can be done at Embody ahead of time or just before their appointment.

Parents or guardians are not required to stay for the appointment but can if either party feels it’s necessary. Girls under 18 who wish to have any bikini waxing or sugaring done must have a female parent or guardian present in the room during their treatment.
